1. You can take the survey interactively from your phone, tablet, or PC. To do so, click on the QR code below or enter the following URL into your browser. NOTE: This is an interactive class survey and will only be available on October 22 from 9:45 AM until 11 AM. Otherwise, use option 2 below.

2. You can take virtually the same survey at your leisure by clicking on “Take the Non Interactive Survey” link. Your responses will be merged with the results of the interactive survey.
